For Fitbit Force, fill out the below form in order to register for a refund check of USD $140.

Fitbit will:

1.	Provide a return kit with instructions on how to return your Fitbit Force.
2.	Receive and verify the returned Fitbit Force. Any non-Force products will be returned to you.
3.	Send a reimbursement check within 2 - 4 weeks after receipt of the Fitbit Force at the Recall center.
